Chart Hero
We developed a chat-style AI tool in the electronic health record that uses large language models (LLMs) to extract key details from a patient chart and generate summaries or responses as prompted. With a tool to quickly retrieve accurate information, care practitioners can spend less time searching through complex charts and more time with patients.
High-Risk Follow Through
We use LLMs to identify patients with overdue care recommendations, such as overdue lung scans, based on text in the electronic health record. This method captures more missed care recommendations than other tools because it can review free-text information in a patient’s chart, whereas other methods are often capable of looking at structured pieces of data only. We then follow up with these patients and their clinical teams to schedule care promptly.

Our homegrown coordn8 platform uses optical character recognition and natural language processing to interpret and organize medical records. By automating repetitive tasks, we free up care teams to spend their time on higher-value work and eliminate some of the records-related hiccups that can slow down care.
Our Approach
At CHTI, we are using AI to fundamentally improve how care is delivered at Penn Medicine. We are embedding it into everyday clinical operations – helping clinicians work more efficiently, identifying patients who need follow-up, and creating more meaningful ways to engage patients in their care. Our work spans predictive models, natural language processing, and emerging generative AI tools, all focused on practical, real-world applications that improve care.
Our approach is people-first. Our AI solutions are designed to take on routine tasks so clinicians and care teams can focus on what matters most: caring for patients, making complex decisions, and building trust. Every solution is co-developed with frontline teams to ensure it meets real needs and improves care in meaningful ways.
Beyond leveraging AI for individual projects, CHTI is advancing a broader, health system–wide AI strategy co-led by Dr. Raina Merchant, chief transformation officer and CHTI executive director. This includes co-defining an AI roadmap, processes, and metrics; exploring different AI tools for potential adoption and partnering on implementation; and educating the Penn Medicine workforce about how AI can support their efforts – and how to use it effectively and responsibly.
