Hospital at Home

ACTIVE PROJECT

Hospital at Home is a safe, patient-centered alternative to traditional inpatient care that provides hospital-level services in the home. It aims to increase hospital capacity, improve patient outcomes, and enhance experience. We had a key role in building the program and are facilitating efforts to expand its reach. Hospital at Home’s forerunner, PATH, was also a CHTI-led program.

Rapid-Access Oncology

ACTIVE PROJECT

Our Rapid-Access Oncology initiative redesigns the earliest phase of cancer care to help newly diagnosed patients connect with a clinical team and begin moving toward treatment sooner. A key component of the OnePenn Breast Oncology program—the foundational program in this initiative—is RN-led televisits that offer patients a touchpoint within 48 hours to answer questions, gather information, and coordinate imaging, referrals, and other next steps before the first physician visit. 

CARe (Connected Approach to Recovery)

ARCHIVE PROJECT

We collaborated on a new model of care for patients recovering from breast reconstructive surgery to lighten the burden that typical post-mastectomy care, often involving multiple clinic visits, placed on these patients and their caregivers. Using remote symptom monitoring and digitally facilitated care coordination, we reduced clinic visits and enhanced patient and provider satisfaction while maintaining high quality of care.

Penn Medicine OnDemand

We co-created a video telemedicine practice to make care available whenever patients need it from wherever they have an internet connection. Virtual visits through Penn Medicine OnDemand are not only convenient, they prevent avoidable visits to the emergency department, where capacity is already constrained.

Heart Safe Motherhood

ARCHIVE PROJECT

Now in use at six health systems, Heart Safe Motherhood replaces postpartum hypertension visits with home blood pressure monitoring, making it easier to monitor maternal health. Patients send blood pressure readings via text message, and care teams respond rapidly when measurements fall outside the normal range. Developed as a CHTI collaboration and implemented through our Way to Health platform, Heart Safe Motherhood has been shown to reduce health complications and make follow-up more equitable.
