Deeply Rooted

Deeply Rooted uses the healing power of nature to promote health equity, well-being, and environmental justice. The collaborative – which is led by the Center for Health Justice in partnership with community development corporations, faith-based organizations, schools, and grassroots greening organizations – has planted more than 1,000 trees, greened more than 1 million square feet of vacant lots, and funded dozens of local initiatives that care for or celebrate nature in neighborhoods in West and Southwest Philadelphia.

IMPaCT

IMPaCT is an evidence-based program that improves patient outcomes by mobilizing community health workers – individuals who are from the communities they serve and offer services such as advocacy, social support, and health coaching. Partnering with the Penn Center for Community Health Workers, CHTI helped create an adaptable business model and toolkits that enabled the program to scale. IMPaCT has supported around 25,000 people in the Philadelphia area.
